A member and his eligible spouse may choose to waive the spouse’s right to
receive the post-1986 survivor benefit in favour of one or more Designated
Beneficiaries (i.e., in the case of remarriage where the member has children
from a previous marriage). To waive the benefit, the member’s spouse must
complete and sign the Spousal Waiver Pre- Retirement Death Benefit (OPTrust
1023) form. However, the member and his/her spouse
may not waive the spouse’s right to receive the pre-1987 survivor benefit. The
eligible spouse will continue to be entitled to receive these benefits.
