Once OPTrust receives the completed Membership Enrolment form
(OPTrust 1005) or Transfer of Membership Between the OPSEU Pension Plan and
the Public Service Pension Plan form (OPTrust 1040) for a new member, we
will mail a Welcome Package to the individual’s home. Depending on the
circumstances – including whether OPTrust has all required information and
whether a buyback cost quote (Agreement to Purchase Credit) is required –
it may take between 2 weeks and 4 months before the member receives a Welcome
Package.
This package includes a summary of the member’s personal data provided at
enrolment, together with copies of key OPTrust publications and details on how
to obtain more information about the Plan. The package also includes second
copies of forms that are normally completed at the time of enrolment. These
additional forms are provided so that the member may make any required changes
to his/her information.
The OPTrust Welcome Package contains:
- A letter welcoming the new member to the Plan
- A statement confirming the member’s personal data provided at enrolment
- It's Your Pension - OPTrust’s Plan booklet
- Your Pension and Buying Back Credit booklet
- Application for Past Service Credit (OPTrust1036) form (enclosed in
Your Pension and Buying Back Credit booklet)
- Agreement to Purchase Credit (OPTrust1017) form (if applicable)
- OPTrust’s Annual Highlights for Members and Pensioners
- OPTions – OPTrust’s member newsletter
- Welcome to the OPSEU Pension Plan [a CD containing an interactive
overview of the Plan]
- Identifying Benefit Recipients (OPTrust 1015) form
- Survivor Benefits & Minor Children fact sheet
- Statement of Spousal Relationship (OPTrust 3007) form
