Employers must complete the following procedures to enroll a new
employee in the OPSEU Pension Plan. The same enrolment procedures should
be followed whether:
- membership is mandatory, or
- membership is optional and the member elects to join
Except in cases of mandatory transfers from the PSPP, employers must
complete OPTrust’s Membership Enrolment form with the employee and submit it
to OPTrust.
- For mandatory enrolments the Membership Enrolment form must be
completed and submitted to OPTrust at the time the member begins
employment.
- For optional enrolments, the Membership Enrolment form must be
completed at the time the member elects to join the Plan.
When the employee has credit in the PSPP and is joining the OPSEU Pension
plan with no break in service, the mandatory transfer provision applies. In this
case, employers must complete the Transfer of Membership form.

Important! Both the employer and member must sign the Membership
Enrolment form, or the Transfer of Membership form.

OPTrust also requires the following forms and documents for enrolling
members. This information may be submitted to OPTrust at the same time
as the Membership Enrolment form or at a later date.
Employers should request that members provide this information to
OPTrust. If the member does not submit the required forms and documents
at enrolment, OPTrust will follow up with the member after processing
the enrolment.
The required documents include:
- Identifying Benefits Recipients form (OPTrust 1015) – Members can also
complete this form online through OPTrust Online Services, a
secure, password-protected section of the OPTrust website, once they
have started to pay contributions to the Plan.
- Statement of Spousal Relationship form (OPTrust 3007)
- Application for Past Service Credit (OPTrust 1036) if applicable.
Please note that timelines apply for submitting an application to buy
back past credit for past service.
- copy of member’s proof of birth (e.g. birth certificate, passport)
- copy of spouse’s proof of birth (e.g. birth certificate, passport), if
applicable
- copy of proof of marriage or common-law status, if applicable. See OPTrust’s

list of documents that are acceptable as proof of the member’s spousal
relationship.

Important! Please send the Membership Enrolment form
or the Transfer of Membership form to OPTrust promptly as soon as the
form is completed. Forwarding this form should not be delayed until the member
has completed the remaining forms and provided the requested documentation.

Employers must ensure that all newly enrolled members of the OPSEU
Pension Plan are set up for correct pension payroll deductions promptly,
starting from the employee’s plan membership date.

Tip: For optional membership, employers may want to begin plan
membership and pension contributions to match the pay period start date.
