Do I need to tell OPTrust if my address, e-mail address or phone number changes?
Yes. When you move or change other contact information, you can use secure e-mail to Contact Us or use Online Services to make the changes yourself. Please make sure OPTrust has up-to-date information for the following:
- e-mail address
- mailing address
- marital status
To change your beneficiary information, including a change in marital status, you can use Online Services or complete a new Identifying Benefit Recipients form.
