What to do if a member dies
If you die while employed, your survivor should contact your employer and your employer will contact OPTrust. Once OPTrust is notified, we will issue a statement outlining the survivor options available.
If your spouse,child or named beneficiary dies you can inform OPTrust by completing a new Identifying Benefit Recipients form or by updating your records on Online Services
