1. Click on the title of the form that you would like to complete.
   If Adobe Acrobat is set up to work with your browser, the form will open inside the browser window. Otherwise, your browser will prompt you to save or open the file; choose Open and click OK to open the form in Acrobat outside of your browser.
   
2. Once the form is open in Adobe Acrobat, fill in your information directly on the form by clicking in the appropriate box and typing.
3. When you have completed the form, choose File > Print from the menu, or click the Print icon on the toolbar. The Print dialogue will appear. In your print dialogue, specify that you want to print 2 copies.
4. Sign one of the printed copies to send to OPTrust. Keep the other copy for your records.
